Apply for an Tournament Permit
This form is used by team coaches and managers to request approval for participating in an Tournament.
Please complete the form and submit it for approval from the the Ice Convenor for each tournament you would like your team to participate in.
You must wait for confirmation before booking the Tournament. After the approval is done the OMHA Rep will issue an approval.
