Personal Information Consent
All parents on your team need to fill out the online Personal Information Consent Form. The form is mandatory for all registered players.

Programs/Qualifications for Managers
All Managers MUST have the following courses done before being rostered.
** The same courses are also mandatory for Coaches and Trainers.

OHF Rowan’s Law Acknowledgement Form

The Rowans law waiver is completed online by parents/guardians, for each player, on the Hockey Canada website during registration. The registrar or OMHA rep will contact the team manager or families directory who have not yet completed the waiver. All waivers must be completed on the website for a team to be rostered with the OMHA.
