Apply for an Exhibition Game Permit
This form is used by team coaches and managers to request approval for participating in an Exhibition Game.
Please complete the form and submit it for approval from the the Ice Convenor for each exhibition game you would like your team to participate in.
You must wait for confirmation before booking the Exhibition game. After the approval is done the OMHA Rep will issue an approval.
