Permission To Skate Request

This form is mandatory if you or your player intends to tryout at another center at a higher level.
See Junior / AAA / AA / A Info for more information on the OMHA's residency rules to find out which other centers you can try out for.

These are the reasons you need an approved PTS form to tryout at another center:

Registration Placeholder
This form will act as a registration placeholder to reserve a spot with Newcastle Hockey while you attend tryouts at another center. You do NOT need to formally register at this time. 

Please note: This registration acts ONLY as a placeholder in the interim and you will still be expected to officially register with Newcastle Hockey immediately if your tryouts are unsuccessful.

Proof of Residency
The approved form that will be emailed to you must be brought to the other center to prove to them you are a resident of NVMHA and are approved to skate.

Let us know if we should save you a spot in Newcastle in the event your tryouts are unsuccessful. This does NOT count as an official Newcastle registration. You will still need to complete an official registration at that time, but a spot will be held for you.
